Employers with 15 or more employees must provide a job accommodation request form upon request to any individual who discloses a disability needing accommodation

The Minnesota Commissioner shall develop and make available a standardized job accommodation request form for employers, employment agencies, and labor organizations to use and distribute

The form must allow employees to identify limitations resulting from their disability and potential reasonable accommodations that could overcome those limitations

Amends Minnesota Statutes section 363A.08, subdivision 6, which governs reasonable accommodation requirements under the state Human Rights Act
