Mandates the Commissioner of Health to develop and make available an immunizations education module covering the risks and benefits of immunizations, with the commissioner determining content, format, and length

Exemption statements must be signed by both the parent/guardian and the healthcare provider (physician, physician assistant, or advanced practice registered nurse) verifying completion of educational requirements, in addition to existing notarization requirements

Authorizes the Commissioner of Health to issue written determinations during disease outbreaks, allowing schools and child care facilities to exclude children with conscientious belief exemptions for the specified disease until they are immunized or qualify for another exemption type

Appropriates an unspecified amount from the general fund in fiscal year 2027 as a one-time appropriation to the Commissioner of Health for developing the immunizations education module
