Prohibits the Commissioner of Human Services from implementing an individualized budget methodology for home and community-based services waivers that determines budgets based on where a person lives; budgets must be based solely on assessed individual needs

Creates a budget exception process for self-directed home care nursing services for individuals who meet specific criteria: assessed to need hospital-level care, receive support range budget E or H, and do not receive community residential or customized living services

Requires home care nursing services under the budget exception to be ordered by a physician, physician assistant, or advanced practice registered nurse, and performed by a registered nurse or licensed practical nurse

Mandates the commissioner develop and implement by July 1, 2026 an online support planning and tracking tool that shows individuals their total available budget, eligible services, and services used, with input from people with disabilities

Effective the day following final enactment, amending the Waiver Reimagine Phase II provisions originally set to take effect January 1, 2027 or upon federal approval
