Reorganizes Minnesota's Medical Assistance provider enrollment requirements by moving existing provisions from section 256B.04, subdivision 21 into new dedicated sections (256B.044 through 256B.0445) covering enrollment, revalidation, suspensions/terminations, payment withholds, and requirements for high-risk and specific provider types.

Requires all individuals employed by or owning high-risk agencies to enroll individually as Medical Assistance providers and obtain a unique Minnesota provider identifier, effective January 1, 2027.

Mandates that high-risk service providers use electronic visit verification systems, obtain signed provision of service verification forms from recipients, and document travel time with start/stop times and locations, effective January 1, 2027.

Expands electronic visit verification requirements to include services provided by commissioner-designated high-risk providers and home and community-based waiver services reimbursed at hourly or minute-based rates.

Updates documentation requirements for home and community-based services to require individual provider identifiers on service records, and makes technical corrections updating statutory cross-references throughout related sections.
