Establishes conditional geriatric release for incarcerated persons aged 55+ who have served at least 10 years or 50% of their sentence, or aged 60+ who have served at least 20 years, if they have a chronic age-related medical condition that substantially diminishes their ability to function in a correctional facility

Creates family caregiver release allowing incarcerated parents of minor children or spouses/partners of incapacitated individuals to petition for release when no other adequate caregiver is available, with a 30-day review process by a multidisciplinary committee

Authorizes conditional release for extraordinary and compelling circumstances comparable to federal standards under 18 U.S.C. § 3582(c)(1)(A), including terminal illness, debilitating injury, pregnancy, advanced age, or extraordinary family circumstances

Requires the commissioner to issue written decisions within 30 days of petition receipt, with applicants permitted to petition the sentencing court for review if deadlines are missed or upon administrative denial

Mandates a publicly accessible database tracking release requests and annual reports to the legislature beginning January 15, 2028, detailing application data and projected cost savings; effective date is July 1, 2026
