Commissioner of Health shall establish a single grant to expand the community care hub model, which connects health care institutions with networks of community-based organizations providing social care services addressing food insecurity, housing instability, transportation access, and health literacy

Eligible applicants must be federally recognized by the Administration for Community Living and CDC, hold contracts with Minnesota health plans, and demonstrate active community-level engagement in health and social care coordination

Grant funds must be used to organize community-based organizations, expand social care services, centralize administrative functions (contracting, payments, referrals, technology, data collection), and create sustainable financial pathways for health-related social services statewide

$8,900,000 appropriated in FY2026 for the community care hub grant, plus $1,000,000 for evaluation and technical assistance; both are onetime appropriations available until June 30, 2030

Commissioner of Health must evaluate the initiative using measures of cost savings, impact, and health outcomes; grantee must report outcomes on forms and timelines established by the commissioner; effective July 1, 2025
