Defines "home care worker" as an individual providing in-home, nonprofessional long-term services to older adults or persons with disabilities, funded through medical assistance programs including home and community-based waivers

Requires the commissioner to submit annual reports by February 15 to legislative committees detailing the number of applicants, grant recipients, total dollar amounts, and outcome indicators

Appropriates funds from the general fund starting in fiscal year 2026, with up to 5% available for administrative costs for both programs
