Parents or guardians must provide certification of a completed physical examination for students in second and eighth grades by the first day of school, with a 90-day grace period if not submitted initially, beginning with the 2011-2012 school year.

Physical examinations must be conducted by a licensed physician, advanced practice registered nurse, or personnel on a statewide registry maintained by the board of healing arts and board of nursing.

Required examination components include health history, hearing evaluation, body weight and vital sign measurements, asthma screening, diabetes screening for eighth graders when appropriate, and immunization record review.

The Department of Health and Senior Services must develop a standardized physical examination form in collaboration with the Missouri State Medical Association and Missouri Osteopathic Association.

Students are exempt from the physical examination requirement if parents or guardians submit written statements citing moral or religious objections, or if they decline the examination.
