Requires group health insurance policies covering more than 25 employees to include coverage for infertility diagnosis and treatment, effective January 1, 2011.

Covered infertility treatments include in vitro fertilization, artificial insemination, embryo transfer, gamete intrafallopian tube transfer, zygote intrafallopian tube transfer, and low tubal ovum transfer.

Limits coverage for in vitro fertilization and related procedures to individuals who have failed less costly infertility treatments and have not undergone more than four completed oocyte retrievals (or two if a live birth resulted).

Requires procedures to be performed at facilities meeting American College of Obstetric and Gynecological guidelines or American Fertility Society minimal standards.

Exempts religious institutions and organizations from coverage requirements if the procedures violate their religious and moral teachings, and excludes supplemental insurance policies from coverage mandates.
