Establishes the "Evan de Mello Reimbursement Program" administered by the Departments of Health and Senior Services and Mental Health to provide financial assistance for transportation and ancillary services for eligible children receiving medical treatment.

Eligibility requires children under 21 years old with severe physical illness or impairments, physician certification of likely medical benefit, need to travel 100+ miles for medical services, and financial inability to pay, with household income not exceeding 350% of federal poverty level.

Provides reimbursement for transportation and ancillary services subject to appropriations, with a minimum per-recipient benefit cap of $5,000.

Establishes the program as a "payer of last resort" and includes subrogation rights allowing the state to recover reimbursed amounts from any liable third parties, insurance settlements, or awards received by parents or recipients.

Requires the departments to promulgate rules establishing an application and review process for eligibility determinations and household income limits by August 28, 2010.
