Requires the Department of Social Services to develop and enforce a policy by September 30, 2010, mandating that employees report suspected controlled substance abuse or fraudulent income/household size reporting under public assistance programs, with failure to report resulting in immediate termination.

Prohibits department supervisors from retaliating against or discriminating against employees who in good faith report suspected fraud or drug use under public assistance programs.

Requires health maintenance organizations to limit co-payments for prescription drugs to the usual and customary retail price, with enrollees only required to pay whichever is lower.

Applies the same co-payment limit rule to health insurers and health carriers offering prescription drug coverage.

Requires the Director of the Department of Insurance, Financial Institutions and Professional Registration to prescribe uniform application forms for group health insurance policies and uniform claim forms for health care providers, with an effective date at least 180 days after adoption.
