Directs the Department of Health and Senior Services to publish annual statistical reports on abortions by gestational age in weekly increments while protecting patient, physician, and facility identities.

Makes voluntary disclosure of reasons for abortion and family planning history optional but requires facilities to make reasonable efforts to collect this information without discouraging women from providing it.

Changes the penalty for knowingly violating confidentiality of abortion records, reports, or documents from a misdemeanor to a Class D felony.
