Repeals sections 191.765, 191.767, 191.769, 191.771, 191.773, 191.775, and 191.776 of Missouri's Indoor Clean Air Act and replaces them with eleven new sections (191.1200-191.1230) effective January 2, 2011.

Prohibits smoking in all enclosed public places including bars, restaurants, health care facilities, educational facilities, gaming facilities, hotels (at least 80% of rooms), and transportation facilities without exception.

Prohibits smoking in all enclosed workplaces without exception, including common work areas, offices, elevators, restrooms, and vehicles; employers must notify existing and prospective employees of this prohibition.

Establishes outdoor smoking restrictions within 15 feet of building entrances and windows, in outdoor arenas and bleachers, public transportation stations, and playgrounds.

Exempts private residences (except child care and health care facilities), tobacco retail outlets, up to 20% of hotel and motel rooms as designated smoking rooms, and outdoor areas of employment from smoking restrictions.

Establishes penalties including infractions for individuals who smoke in prohibited areas and fines up to $50 for first violations by business owners, up to $100 for second violations, and up to $500 for subsequent violations within one year; allows license suspension or revocation.

Prohibits retaliation against employees or customers for exercising rights under the act, classifying violations as Class A misdemeanors.
