Mandates tanning facilities provide written warning statements to customers covering risks including eye damage, burns, premature aging, skin cancer, and medication interactions; requires customer signatures before initial use and contract renewal.

Establishes operational requirements including trained operators on-site, provision of sanitized protective eyewear, timers accurate within plus or minus ten percent, manual radiation termination controls, and interior temperature not exceeding 100 degrees Fahrenheit.

Prohibits persons under age 16 from using tanning devices; requires written parental or guardian consent for users under age 18 obtained in person at the facility.

Establishes a Tanning Facility Fund through licensing fees set to cover only administrative costs; violations of the bill's provisions constitute a class C misdemeanor.
