Allows adopted persons age 18 and older born in Missouri to obtain uncertified copies of their original birth certificates by filing an application with the state registrar and providing proof of identification.

Requires adopted persons to agree in writing to abide by any contact preferences stated on a form attached to their original birth certificate.

Permits birth parents to voluntarily complete medical history forms and contact preference forms indicating whether they wish to be contacted by adoptees, prefer contact through an intermediary, or do not wish to be contacted.

For adoptions completed before August 28, 2010, restricts release of original birth certificates to cases where the birth mother is deceased, or requires the state registrar to attempt contact with the birth mother within three months to obtain consent.

Establishes that medical history information from birth parents shall be provided to adopted persons in redacted form (with identifying information removed) and contact preference forms shall receive the same level of confidentiality as original birth certificates.
