Requires institutions to provide unmarried mothers with a paternity acknowledgment form that can be completed by the child's mother and father.

Designates completed paternity acknowledgment forms as non-public records, but allows copies to be provided upon request to the mother, father, their attorneys, the child, guardian ad litem, child's attorney, and state and federal child support agencies.

Requires a fee established by the Department of Health and Senior Services for providing copies of the acknowledgment form to eligible parties.

Provides civil and criminal immunity to institutions and their staff for providing the paternity acknowledgment form and related information.

Establishes that intentional misidentification of a parent on the form constitutes perjury under Missouri law.
