Requires the Department of Mental Health to contract with an independent third party to develop a transition plan for residents of state developmental disabilities facilities, including ICF/MR facilities and congregate waivered group homes.

Plan must recommend for each resident the most integrated settings appropriate to their needs, the cost of providing those services, and a timetable for transition.

Requires separate identification of recommendations for court-committed residents versus non-court-committed residents, with discussion of how to address cases where institutional settings are recommended.

Plan must include recommendations for permanent state employees at these facilities, covering other employment opportunities, training availability, and assistance needed.

Plan must address alternative uses for state-owned facility property with input from local residents and identify total costs, cost savings, funding sources, implementation obstacles, and a proposed implementation schedule; report due by December 31, 2010.
