Requires a written contract signed before a notary public and witness between legal embryo custodians and recipient intended parents prior to embryo transfer, which transfers all legal rights and responsibilities for the embryo and any resulting child.

Mandates that embryo donors be screened and tested negative for infectious agents on the FDA's Complete List of Donor Screening Assays prior to embryo creation, with exemption for embryos existing before the law's effective date.

Allows courts to waive technical requirements in embryo transfer proceedings to promote stability of transfers and protect interests of children born from such transfers.

Terminates all parental rights and responsibilities of past or present legal embryo custodians and gamete donors in any child resulting from a completed embryo transfer contract, vesting full rights and responsibilities in the recipient intended parent.
