Creates the Missouri and Midwest High-Speed Rail Commission composed of 15 members: 12 public members appointed by the governor and 3 ex officio members (directors of transportation and economic development, and highways and transportation commission chairperson).

Requires appointed public members to be Missouri residents with expertise in local government, transportation policy, infrastructure financing, and railroad projects, reflecting geographic diversity across all state regions.

Mandates the commission issue a comprehensive report by March 20, 2011 recommending governmental structure, legislation, funding sources, and integration strategies for a high-speed rail system capable of trains traveling over 150 miles per hour.

Requires a follow-up report by February 1, 2012 detailing status of prior recommendations and revised proposals based on public and stakeholder input.

Establishes commission meetings at least monthly until April 2011, then quarterly thereafter, with authority to accept public and private funding and to collaborate with the Department of Transportation on federal high-speed rail planning requirements.
