Allows adopted persons age 18 and older born in Missouri to obtain uncertified copies of their original birth certificate by filing a written application with proof of identification to the state registrar.

For adoptions completed before August 28, 2010, requires the state registrar to make reasonable efforts to contact the birth mother within three months to obtain consent before releasing the original birth certificate; certificate is only released without consent if the birth mother is deceased.

Requires birth parents to complete medical history forms containing information on congenital history, psychosocial history, chronic diseases, infectious diseases, allergies, pregnancy and birth history, and family deaths that may affect medical history.

Allows birth parents to submit contact preference forms indicating whether they wish to be contacted by the adoptee, prefer contact only through an intermediary, or do not wish to be contacted; these forms attach to the original birth certificate.

Applies to stepparent adoptions only if a new birth certificate is specifically requested by the adoptive parent or stepparent, and extends recognition of foreign country adoptions with proper immigration documentation.
