Establishes the "Comprehensive Tobacco Control Trust Fund" in the state treasury to receive strategic contribution payments from the master settlement agreement beginning in fiscal year 2011 and continuing perpetually, with funds dedicated only to evidence-based prevention and cessation programs.

Creates the "Commission for Comprehensive Tobacco Control" within the department of health and senior services, composed of thirteen members including representatives from cancer, heart disease, and lung disease organizations, the Missouri State Medical Association, nursing profession, smoking prevention advocacy, local medical societies in Kansas City/St. Louis/Greene County, and three state officials.

Commission members appointed by the governor serve two-year terms subject to reappointment, with initial appointees split evenly between one-year and two-year terms, and service is voluntary with reasonable expense reimbursement for non-state employees.

Prohibits commission members from being employees of or having any financial interest in tobacco companies or organizations with financial ties to tobacco companies.

Requires the commission to submit a report by December 15, 2010, detailing program guidelines and administration recommendations, followed by annual reports by December 15 each year evaluating funded programs based on accountability standards.
