Physicians must complete individual abortion reports for each procedure including the specific method used (vacuum aspiration, D&E, medication abortion drugs, etc.), reasons for the abortion, and family planning methods used at time of conception.

Physicians providing post-abortion care must submit complication reports within 45 days including the abortion date, facility location, and nature of complications diagnosed or treated.

The state department of health and senior services must collect all reports, collate data, and publish annual statistical reports by gestational age without identifying patients, physicians, or facilities.

Information collected shall be kept confidential and used only for statistical purposes, though public health officers may inspect records.

Knowingly violating the confidentiality of abortion records or reports is changed from a misdemeanor to a class D felony.
