Requires all personnel at hospitals, ambulatory surgical centers, and health care facilities to wear identification badges displaying licensure status, recent photograph, first name, title, and facility name.

Mandates employee titles appear in block type on a one-half inch strip at the bottom of badges, with specific titles for physicians ("Physician"), nurses (including "Registered Nurse," "Licensed Practical Nurse," "Certified Nurse Practitioner," and others), and other titles to be determined by department rule.

Allows health care providers to display additional specialty or designation information after their name on the badge.

Exempts personnel from wearing badges while delivering direct care to consumers when not clinically feasible.

Provides existing employees until August 28, 2016, to comply with new badge requirements, and authorizes the Department of Health and Senior Services to establish implementing rules.
