Allows ambulance service providers to submit claims for unpaid healthcare services (minimum $50) that are outstanding for more than 90 days to a claim clearinghouse for collection via income tax refund or lottery prize setoff.

Requires providers to notify patients in writing before submitting claims, informing them of the right to contest the claim's validity or amount through a review officer within 30 days.

Establishes a two-tier appeal process where patients can request review by a provider-appointed review officer, and if dissatisfied, appeal to a provider-appointed appeals committee.

Imposes a collection assistance fee of $7 to the Department of Revenue and $17 to the claim clearinghouse (total $24) for each claim processed, recoverable from the eligible claim amount.

Allows patients to request a hearing with the Department of Revenue within 30 days of setoff notification to contest the claim, with appeals available under Chapter 536 procedures.
