Requires all certificate of insurance forms to be filed with the director of the Department of Insurance, Financial Institutions and Professional Registration before use, and prohibits altering or modifying filed forms.

Mandates that each certificate of insurance contain a specific statement indicating it is issued for information only and confers no rights upon the certificate holder or alters the referenced policy's coverage, terms, exclusions, or conditions.

Prohibits any person from demanding, requesting, preparing, or issuing a certificate of insurance containing false or misleading information or purporting to alter, amend, or extend policy coverage.

Establishes that a certificate of insurance cannot confer new rights to certificate holders beyond what the referenced policy expressly provides, and cannot contain references or opinions on contracts other than the insurance contract itself.

Authorizes the director to issue administrative orders for violations as level two violations under state law, and permits rulemaking to implement these provisions.
