Department of Mental Health must submit a transition plan by December 31, 2012, to shift residents of state developmental disabilities facilities to the most integrated and least restrictive settings appropriate to their needs.

Plan must contract with an independent third party to conduct a study and develop recommendations for each resident, including the most appropriate setting, cost of community services, barriers to community placement, and transition timeline.

All long-term admissions to state-run intermediate care facilities for the mentally retarded cease upon the effective date of this section.

Plan must include contemporary best practices, research data on residential settings, recommendations for affected state employees, alternative uses for facility property, total costs and savings, funding sources, and legal obstacles to implementation.

Implementation schedule targets January 1, 2018, as the goal for shifting provision of services to the community for every resident; any plans to build or renovate state facilities initiated after August 28, 2011, are frozen until plan completion.
