Requires physicians, chiropractors, hospitals, dentists, and other licensed healthcare providers to furnish patient medical records within 15 business days of a written request, or notify the requester of a delay with the reason, with a maximum timeline of 90 calendar days.

Allows providers to charge fees for copying (maximum $17.05 plus $0.40 per page), postage/packaging, and notary services (maximum $2), with fees automatically adjusted annually on February 1st based on the medical care component of the Consumer Price Index.

Providers who fail to deliver medical records within required timeframes or fail to notify of delays forfeit the right to collect copying fees and are liable to requesters for three times the allowable copying fee if more than 30 days delinquent.

Requires providers to furnish statements of medical expenses incurred for services within 15 business days of request, with providers liable for three times the copying fee if they fail to comply.

Protects providers from liability for good faith disclosure of patient records as required by this section.
