Requires all hospitals and health care facilities to mandate that personnel wear identification badges displaying licensure status, including employee photograph, name, title, and facility name.

Specifies employee titles must appear in block type on a one-half inch tall strip at the bottom of the badge, with standardized titles for physicians, registered nurses, and licensed practical nurses; other titles determined by department rules.

Permits exceptions allowing personnel to omit badges while delivering direct care if not clinically feasible, and allows concealment of last names when caring for consumers exhibiting symptoms of irrationality or violence.

Sets compliance timeline requiring all new employees to have compliant badges immediately, while existing employees' badges must comply within five years or upon replacement, whichever comes first.

Authorizes the Department of Health and Senior Services to promulgate implementing rules subject to chapter 536 provisions.
