Rejects "certification of live birth" documents that lack the birthing facility name, street address of birth location, or signature of the certifying administrator, physician, midwife, or other authorized person

Mandates submission of a "long form birth certificate" containing facility name, address, and authorized signature if such a document exists

Requires the Secretary of State to publish the submitted proof of birth, including any long-form birth certificate, on the internet for public viewing
