Department of Mental Health must submit a transition plan by December 31, 2012, for moving residents of state developmental disabilities facilities and intermediate care facilities for the mentally retarded to the most integrated community settings appropriate to their needs.

Plan must include individual assessments for each resident identifying recommended community services, costs, barriers to community placement, and transition timelines.

Plan shall address contemporary best practices, research on residential settings and quality of life, employment options for displaced state workers, alternative uses for state facilities, total costs and savings, and required service development including crisis intervention.

Implementation schedule shall target shifting all resident services to the community by January 1, 2018.

All long-term admissions to state intermediate care facilities for the mentally retarded cease upon the effective date, and any facility construction or renovation plans started after August 28, 2011, are suspended until plan completion.
