Repeals and reenacts section 374.184, RSMo, to expand the director of the Department of Insurance, Financial Institutions and Professional Registration's rulemaking authority regarding health insurance forms.

Requires the director to prescribe uniform claim forms for health care providers based on UB-82 forms for hospital claims and HCFA 1500 forms for physician claims, after consulting with providers and holding a public hearing.

Requires the director to prescribe uniform application forms for group health insurance policies (other than small employer plans) after consulting with insurers, health services corporations, health maintenance organizations, and insurance producers, and after a public hearing.

Allows insurers and providers to modify uniform billing documents by contract if both parties agree such modifications streamline claims processing, but prohibits insurers from refusing to contract with providers who decline to accept modifications.

Establishes an effective date of no less than 180 days after rule adoption, after which insurers and providers must use only the director-prescribed forms, except that UB-82 and HCFA 1500 forms remain permissible.
