Department of Mental Health must submit by December 31, 2013 a plan for transitioning residents of state developmental disabilities facilities and intermediate care facilities for the mentally retarded to the most integrated and least restrictive settings appropriate to their needs.

Plan shall include individual assessments recommending appropriate community settings, costs of community services, barriers to community placement, and transition timelines for each resident.

Plan must address contemporary best practices, quality of life research, nationwide trends, employment options for displaced state workers, alternative uses for state facility property, total costs and savings, federal funding sources, and needed service development.

Implementation schedule shall target shifting all residents to community services by January 1, 2019.

Long-term admissions to state-run intermediate care facilities for the mentally retarded are prohibited effective immediately, and any facility construction or renovation projects started after August 28, 2012 are frozen until the transition plan is completed.
