Allows adopted persons age 18+ born in Missouri to obtain their original birth certificate after August 28, 2012, unless birth parents object using a form provided by the court.

Requires birth parents to be given the opportunity before adoption finalization to indicate whether they object to disclosure of the original birth certificate and to complete medical history and contact preference forms.

Enables adopted persons whose birth parents objected to request the Department of Social Services, child-placing agencies, or courts make reasonable efforts to locate birth parents and request their consent to release the original birth certificate.

Allows prospective adoptive parents and birth parents to enter into written post-adoption contract agreements to allow contact after adoption, which courts must enforce unless not in the adoptee's best interest.

Requires the Missouri Supreme Court to develop a standardized adoption finalization form by July 1, 2013, that includes a checklist verifying all required documents and procedures have been completed and reviewed by the judge.
