Changes the effective date of comprehensive vision examination requirements from July 1, 2008 to July 1, 2013 for students referred through screening programs.

Modifies eye screening requirements to occur once before completion of kindergarten, once before completion of first grade, and again before completion of third grade, instead of a limited pilot period.

Requires students who fail eye screenings to receive mandatory comprehensive vision examinations from an optometrist or physician, with results reported to parents and the Department of Health and Senior Services.

Extends the sunset date of the Children's Vision Commission from June 30, 2012 to June 30, 2016, and requires the commission to submit a compliance report to the legislature by December 31, 2016.

Maintains parental opt-out provisions allowing parents or guardians to request in writing that their child be excused from vision examinations or screenings.
