Requires all tanning facilities to obtain a license from the Department of Health and Senior Services and display it conspicuously.

Mandates tanning facilities provide customers with written warning statements covering risks including eye damage, burns, premature aging, skin cancer, and medication interactions, requiring customer signatures before initial use and contract renewal.

Requires facilities to post an 11" x 17" danger warning sign near entrances and within three feet of each tanning device with specific lettering requirements.

Establishes operational requirements including trained operators on duty, sanitized protective eyewear for all users, timers with ±10% accuracy, manual radiation termination controls, manufacturer-recommended exposure limits, and interior temperature not exceeding 100°F.

Prohibits persons under 16 from using tanning devices and requires written parental consent for users under 18; establishes a Tanning Facility Fund to cover licensing costs through fees set by the department.

Exempts photo-therapy devices used under physician supervision, personal residential use, and devices intended for non-tanning purposes.

Violations are classified as class C misdemeanors.
