Requires group health insurance policies covering more than 25 employees to include coverage for infertility diagnosis and treatment, effective January 1, 2013, including procedures such as in vitro fertilization, artificial insemination, and embryo transfer

Coverage for in vitro fertilization, gamete intrafallopian tube transfer, and zygote intrafallopian tube transfer is limited to individuals who have exhausted less costly medically appropriate treatments and have not undergone more than four completed oocyte retrievals (or two retrievals if one resulted in a live birth)

Procedures must be performed at medical facilities conforming to American College of Obstetric and Gynecological guidelines or American Fertility Society minimal standards for in vitro fertilization

Religious institutions and organizations, and entities sponsored by them, are exempt from providing this coverage if the procedures violate their religious and moral teachings

Exempts supplemental insurance policies, including Medicare supplements, long-term care policies, and short-term major medical policies of six months or less, from coverage requirements
