Mandates that if technology does not permit automatic adjudication of combination dosage co-payments, health carriers must provide reimbursement forms to patients.

Allows insureds to fill mail order prescriptions at any network participating retail pharmacy if that pharmacy offers a price comparable to the mail order pharmacy.

Prohibits health carriers from imposing different co-payment fees or other conditions on insureds who choose network participating retail pharmacies versus designated mail order pharmacies.
