Director of the Department of Insurance, Financial Institutions and Professional Registration shall prescribe uniform claim forms for health care providers and uniform application forms for group health insurance policies by rule.

Uniform claim forms shall be based on the UB-82 form for hospital claims and HCFA 1500 form for physician claims, as modified by the National Uniform Billing Committee or federal Health Care Financing Administration.

Director's adoption of uniform claim and application forms does not prevent insurers, health services corporations, or health maintenance organizations from requesting additional information during claims investigations or insurance applications.

Insurers and providers may contractually modify the uniform billing document if both parties agree such modifications streamline claims processing, but a provider's refusal to agree cannot be grounds for denying a contract.

Rules adopted must specify an effective date of at least 180 days after adoption; insurers must then use only prescribed forms and cannot require applicants to complete differing forms.
