Requires group health insurance policies covering more than 25 employees to include coverage for infertility diagnosis and treatment, effective January 1, 2014, including procedures such as in vitro fertilization, artificial insemination, and embryo transfer

Coverage for advanced procedures (in vitro fertilization, gamete intrafallopian tube transfer, zygote intrafallopian tube transfer) requires the individual to have exhausted less costly medically appropriate treatments and permits a maximum of four completed oocyte retrievals (or additional retrievals if a live birth results)

Procedures must be performed at medical facilities conforming to American College of Obstetric and Gynecological guidelines or American Fertility Society minimal standards

Exempts religious institutions and organizations, and entities sponsored by them, from coverage requirements if the procedures conflict with their religious and moral teachings

Does not apply to supplemental insurance policies, including Medicare supplements, accident-only policies, or short-term coverage of six months or less
