Establishes baseline eligibility requirements for MO HealthNet coverage including Missouri residency, valid Social Security number, U.S. citizenship or qualified alien status, and pregnancy verification for pregnant women applicants.

Requires annual eligibility reviews for all MO HealthNet participants effective January 1, 2014, and mandates that applications include all required information and documentation per 42 CFR Section 435.907.

Extends MO HealthNet coverage effective January 1, 2014, to pregnant women, children under 19, CHIP-eligible children, and uninsured women using income eligibility standards based on federal poverty levels and AFDC income standards converted to MAGI equivalents.

Creates alternative benefit package coverage for adults aged 19-64 with household income at or below 100% of federal poverty level, with full benefits available only to those meeting "medically frail" criteria including serious mental illness, substance use disorders, or serious medical conditions.

Requires the Department of Social Services to establish screening processes with mental health and health departments to identify medically frail individuals and to submit necessary state plan amendments to Centers for Medicare and Medicaid Services for program implementation.
