Repeals and reenacts section 208.146, RSMo, to establish the "Ticket to Work Health Assurance Program" for employed disabled individuals under the federal Ticket to Work and Work Incentives Improvement Act of 1999.

Extends eligibility to disabled workers meeting Social Security income definitions who have earned income, asset limits, and gross income not exceeding 250 percent of federal poverty level, with earned income between 250-300 percent excluded from the calculation.

Establishes tiered premium requirements for participants with gross income exceeding 100 percent of federal poverty level, ranging from 4 to 6 percent based on income brackets.

Allows participants to establish medical savings accounts and independent living accounts (limited to $5,000 per year each) using earned income, and requires employers to offer health insurance participation when cost-effective.

Sets program expiration date to August 28, 2019, and requires participants to report income or household changes within 10 days.
