Creates the "Brain Injury Fund" in the state treasury under the Department of Health and Senior Services to receive judgments, federal grants, private donations, and other designated moneys.

Authorizes fund expenditures for transition, integration, and provision of community-based services including comprehensive brain injury day rehabilitation therapy programs, home and community support, and social and educational activities for individuals with brain injury and their families.

Specifies that unexpended balances in the Brain Injury Fund at the end of any biennium shall not be transferred to the general revenue fund.

Imposes a $2 surcharge as costs in all criminal cases, county or municipal ordinance violations, and traffic law violations, with the surcharge not waived except when proceedings are dismissed or costs are paid by the state, county, or municipality.

Directs court clerks to collect the $2 surcharge and remit it to the state treasury for credit to the Brain Injury Fund.
