Repeals and replaces sections 188.015 and 188.030 of Missouri law regarding abortion definitions and restrictions.

Prohibits abortion of viable unborn children except when necessary to avert the pregnant woman's death or serious risk of substantial and irreversible physical impairment of a major bodily function, explicitly excluding psychological or emotional conditions.

Requires physicians to determine gestational age before abortion and, for pregnancies of 20 weeks or more, perform viability testing and obtain written certification and agreement from a second independent physician before performing viable fetus abortions.

Establishes that violations constitute a Class C felony with imprisonment of at least one year and fines of $10,000 to $50,000, with potential license revocation for physicians and license suspension or revocation for hospitals and ambulatory surgical centers.

Requires the Department of Health and Senior Services to conduct minimum four inspections annually of abortion facilities, with authority to immediately close facilities that pose immediate serious risk to patient health or safety.
