Defines "tanning device" as equipment emitting electromagnetic radiation between 200-400 nanometers used for skin tanning, including sunlamps, tanning booths, and tanning beds.

Defines "tanning facility" as any location or business providing access to tanning devices for a fee, membership dues, or other compensation.

Directs the Department of Health and Senior Services to develop a standard consent form by rule for use by all Missouri tanning facilities.

Establishes penalties of $100 for first violation, $250 for second violation, and $500 for each subsequent violation, with each unauthorized use by a minor constituting a separate offense; enforcement begins only after the consent form rule is adopted.
