Allows minors to petition a court for a judicial waiver of parental consent if they demonstrate they are sufficiently mature and well-informed, or if they are victims of abuse or obtaining parental consent is not in their best interest.

Requires courts to schedule hearings within 5 days of petition filing and permits courts to require mental health evaluations before proceedings to assess the minor's maturity and decision-making capacity.

Creates a mandatory consent form that must include detailed descriptions of abortion procedures, risks and hazards, and requires initials and signatures from both the minor and parent or guardian, notarized by a notary public.
