Requires entities that provide pregnancy-related services and advertise prenatal sonography, pregnancy tests, or pregnancy options counseling to disclose whether they offer contraceptive drugs/devices, practitioner consultations, adoption services/referrals, and abortion services/referrals

Exempts entities from disclosure requirements if they employ at least one licensed physician, nurse practitioner, physician assistant, registered nurse, or nurse midwife at all public-facing locations and have such a practitioner onsite whenever medical services are provided

Mandates written notice be conspicuously placed at entrances, waiting areas, websites, and advertisements in language a reasonable person can understand before accepting services

Prohibits entities from disclosing patron health information to any other person without written authorization and requires entities to provide free copies of requested health information upon request

Establishes administrative penalties ranging from $250 to $1,000 for violations, with additional penalties up to $5,000 if violations continue beyond seven days and up to $10,000 per week for violations continuing beyond fourteen days
