Requires the MO HealthNet division and Department of Health and Senior Services to collaborate on coordinating goals and benchmarks to reduce diabetes incidence, improve diabetes care, and control complications in Missouri.

Mandates submission of biennial reports by January 1st of odd-numbered years covering diabetes prevalence and financial impact, assessment of existing programs, coordination between agencies, detailed action plans with benchmarks, and budget blueprints for consideration by the General Assembly.

Limits reporting requirements to existing diabetes information and programs within each agency unless unobligated funding is available for new research, data collection, or other reporting needs.

Establishes the "Show-Me Extension for Community Health Care Outcomes (ECHO) Program" to be managed by the University of Missouri in collaboration with the Department of Health and Senior Services.

Directs the ECHO Program to use telehealth technology to expand treatment capacity for chronic and complex diseases in rural and underserved areas by disseminating best practices from medical experts to local primary care providers and utilizing community health care workers to improve patient compliance and outcomes.
