Requires child care facilities to obtain a written license from the Department of Health and Senior Services to establish, maintain, or operate, except for specific exemptions listed in the statute.

Exempts facilities caring for four or fewer unrelated children, court-appointed guardians, occasional personal guests (up to 90 days), boarding schools, hospitals, religious organizations, mental health facilities, and nursery schools from licensure requirements.

Prohibits exempted facilities from receiving any state or federal funds for providing child care, except for federal Child and Adult Care Food Program funds.

Requires all child care facilities to disclose their licensure status to parents or guardians of children in their care.

Prohibits unlicensed child care facilities from representing themselves as licensed to parents or guardians.
