Establishes a secure online central registry administered by the Department of Health and Senior Services for citizens to store advance health care directives and allow authorized health care providers access to them, subject to appropriations.

Allows adults to submit advance health care directives, declarations, and revocations to the registry with required notarization and a filing fee not to exceed $10.

Requires all registry data to remain confidential and exempt from public records requests under chapter 610.

Grants the Department authority to promulgate rules determining registry access (physicians, health care providers, declarants, and legal representatives), procedures for anatomical gift declarations, annual reminders to users, and filing fees.

Provides that failure to register documents or notify the department of revocations does not affect the validity of documents or revocations that meet statutory requirements.
