Mandates contracting entities maintain a website or toll-free number identifying the insurance carrier used to reimburse participating providers for covered dental services.

Requires contracting entities to identify any third parties granted access to a participating provider's dental services upon request, provided in writing or by email.

Establishes that third parties purchasing access to a provider's discounted rates must comply with the original provider contract terms, including payment rates and methodology, unless the provider agrees otherwise.
